Budget management and service quality in older adult care: A systematic review of critical factors, challenges, and emerging gaps (2020–2025)

Abstract

This study examines the available scientific evidence on the relationship between budget management and service quality in institutional elderly care facilities. The objective was to identify the main trends, challenges, and gaps that influence the efficiency of public spending and its impact on the well-being of older adults. Methodologically, an analytical literature review was conducted based on 30 scientific and technical documents published between 2020 and 2025, retrieved from academic databases such as Scopus, PubMed, SciELO, and ScienceDirect, applying rigorous inclusion criteria to ensure methodological quality. The findings reveal that the shortage of specialized personnel, limited infrastructure, bureaucratic administrative processes, insufficient investment in mental health programs, and weak intersectoral coordination are critical factors that negatively affect service quality in elderly care. In addition, best practices related to results-based budget planning, human capital development, and cross-sector coordination were identified as effective strategies for improving service delivery. In conclusion, budget management emerges as a strategic pillar for ensuring comprehensive and high-quality care for older adults. The study highlights the need for public policies focused on efficiency, equity, and person-centered care to sustainably address the challenges posed by population aging.
